in the Resource Directory

County Map Search

1. On the map, click on the county in which you wish to locate health care providers/services. The colors selected for the counties are for visual purposes only. They do not represent epidemiological trends of signify characteristics of services.

2. After a county is selected, the services available for that county that are currently listed in our database will be displayed.

These counties are also listed under certain categories. Counties are listed in this way based on the description of the topics and services that they provide.

  • Select a provider or agency by clicking on the agency/provider's name. This will give you a complete description of the agency, which includes a mailing and physical address, telephone and fax number, as well as any special eligibility.
